I use a combination of Microsoft Publisher and Paint 3D to make mine. I also occasionally need to use an online image compressing service.
I have a header template set up and ready to go since I like to change mine up periodically. If you would like some additional help, or just want to let someone else do the hard work, let me know. :)

For Tattoos:
Basically, I surf the web for creative commons licensed images, or maybe even draw and scan something of my own. For tattoos, you want simple images with large blocks of color because at 25x25 pixels you're going to lose most of your detail.
Then I use (primarily) Publisher and Paint 3D to manipulate the image(s) (color processing, crop, rotate, etc), and then put the final image on a transparent background and reduce the size to 25x25 pixels using Paint 3D.

For Headers:
I use the same general process, but I use the template I already have set up in Publisher to get everything the right size. Paint 3D is an intermediate step here, usually used to crop certain elements from other images.
