SS Feedback Form Submitted, Now Gone?

So I had filled out my SS Feedback form at the beginning of the month. I just got a little popup reminding me to do it so I went to check, and now the feedback form is all blank.
Does this mean I have to do redo it, or does it always blank out after it's been submitted? I've already deleted all the notes I had kept about the presents I received.... -
Check now. We have been having some issues with it saving as the wrong month.Need to contact me? Read this first. Only send me a PM for PayPal issues or if I ask you to. Otherwise, make a forum post. You will get a better faster answer by making a post.
