I've been playing for almost a year now... And i have yet to understand how to create my own tattoo. Could anyone help me out? Thanks.

Find the 'tattoo registry' on your home page. Open that, upload your image (max 25 x 25 pixels).
Open the tack page for the horse you want to tattoo, and at the bottom open the tattoo manager, and select where you want to place your tattoo.

Rebel, Ammit just recently changed the tattoo system, so what Goliath explained might be easier than the old system, if that's what you were stuck on.

I just dont understand how to make or get the actual image that I need

You can design your tattoo on a drawing app of choice or an image you already have.
Save it to you phone, tablet or pc. To resize it to 25x25 I use http://resizeimage.net
Click download once you have resized it.
Go to the tattoo registry and click select Image
Select the new 25x25 Image
The press upload and it should update :)

If you still don't understand let me know if you use Phone or Pc etc and will try and do a quick tutorial :)
